By completing this form, you consent to the collection, use, and processing of your personal information and sensitive personal information by DigiPlus Foundation Inc. for the purpose of screening, evaluating, verifying, and administering your application under the AlagangPlus Medical Assistance.

To manage the volume of applications, DigiPlus Foundation may engage an accredited third-party application reviewer as an authorized Personal Information Processor (PIP) to assist in evaluation. All third-party reviewers are bound by strict Data Processing and Non-Disclosure Agreements in compliance with the Data Privacy Act of 2012 (RA 10173) and NPC Circular 2020-03, requiring them to implement organizational, physical, and technical security measures and prohibiting secondary use.

In accordance with the DigiPlus Data Retention Policy, your application records and supporting documents will be retained as follows:

• Unsuccessful Applicants: Retained securely for twelve (12) months following the final selection announcement to accommodate any audit, verification, or appeal processes, after which all digital files will be securely purged and physical copies shredded.
• Awarded Patients: Retained throughout the duration of the medical assistance and for a period of five (5) years post-treatment to satisfy regulatory, tax (BIR), and corporate governance auditing requirements.

Your information will not be shared with any other third party beyond what is described in this notice without your separate consent, except where required by law.

Your rights as a data subject include the right to be informed, to access your personal data, to object to its processing, to request correction or erasure, to data portability, to withdraw your consent at any time without retroactive penalty, and to lodge a complaint with the National Privacy Commission (NPC).
